Request Your Medical Records Online with HealthMark
Requesting medical records can be overwhelming. HealthMark Group is there to help and streamline the process to get your records efficiently.
How to request records: Requests can be submitted through HealthMark’s Request Manager. Follow these steps:
Visit the HealthMark Request Manager: Go to HealthMark’s Request Manager.
Log In: Enter your login information to access your account.
Submit Your Request:
Select “Submit Request” from the menu options.
Fill out all required fields to provide authorization directly to HealthMark.
Processing: Your medical record request will be processed.
Notification: Once your request is complete, you will receive a notification by mail or email, and your records will be available for download.
Timeline to process your request: Your record requests are usually processed within 8 business hours. This means that one or two business days after you submit your request, your records will be sent directly to your inbox.

Other Ways to Request Your Records
For quick and convenient access to your health information, use our online Patient Portal. Please note that the portal does not include your entire medical record; for instance, visit notes and records from other providers may not be available there.
If you need copies of information that is unavailable on our Portal, follow the instructions above to request records through HealthMark. You can also fill out our paper request form, located on our Patient Forms website page. You can submit your request in-person, or by fax or mail:

Your records will be released as specified by you in the Authorization form. Once processed, you will receive a notification via email or standard USPS delivery with instructions on how to retrieve your records. To expedite the delivery of your records, please provide your email address on your authorization.
Check the status of your request
To check the status of a previously submitted request please contact HealthMark Group directly by phone at 800-659-4035 or by email at status@healthmark-group.com.
Outside Requestors
Providers: STAT and urgent requests for records should be directly faxed to 817-391-1070 for processing. Sending these requests to HealthMark will cause the request to be delayed. If you are sending records to us, these should also be sent by fax to 1-877-878-7146—not to HealthMark.
Payers and institutional requestors: Please use HealthMark Group’s Medrelease tool to request copies.
