  • Access up-to-date information about your provider and the hospital they admit to

  • Receive referrals to specialists experienced in treating your condition

  • Get timely responses to your questions or information requests

  • Suggest changes to our policies or services

Patient Responsibilities

  • As a patient, you have responsibilities to:

  • Collaborate with your providers to make decisions about your healthcare

  • Communicate openly; let your provider know if you don’t understand your treatment or how to manage your condition

  • Provide accurate and complete information to your provider and their staff

  • Follow the agreed-upon directions and advice from your provider

  • Inform your provider immediately of any unexpected problems or symptoms

  • Consult your provider for referrals to non-emergency specialists or hospital care

  • See the specialists your provider refers you to

  • Present your insurance card and driver’s license before your appointment

  • Read and understand your insurance plan and benefits

  • Pay any applicable copays, coinsurance, and deductibles that are your responsibility
